- Evaluates and treats patients with different diagnosis in accordance with physician’s orders.
- Initiates skilled physical therapy treatments in accordance with physician’s orders.
- Supervises physical therapist assistants and rehab technicians.
- Completes and maintains required documentation such as evaluations, progress notes, daily notes, recertifications, and discharge summaries for patients on caseload.
- Identify and meet patients’ goals and needs.
- Offer cost-effective treatments that help improve clients’ motion and mobility.
- Reduce the need for medications and provide alternatives to surgery.
- Develop care plans using a variety of treatment techniques.
- Create fitness- and wellness-oriented programs tailored to patients’ specific needs.
- Provides quality, personalized and evidence-based care, and proven interventions.
- Motivates patients during treatment to help them function optimally.
- Promotes patients’ healthy lifestyle by improving strength, flexibility, balance, and coordination.
- Consults and practices with other health professionals regarding special needs of patients to enhance rehabilitation or maintenance of function.
- Provides input for the Care Planning meetings and attends during absences of the DOR.
- Maintains statistical records of residents’ treatments.
- Assists with in-service training programs upon request.
- Submits requisition for supplies needed in the treatment of residents to DOR.
- Assumes all duties of the Director of Rehabilitation, if applicable, during absences including supervision of rehab technicians and responsibility for all necessary documentation.
- Arrives to work on time, regularly, and working as scheduled, responsible for own transportation.
- Observes and practices confidentiality of resident information and privacy when performing job duties.
- Recognizes and follows the dress code of the facility including wearing name tag at all times.
- Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.
- Attends facility meetings, staff meetings, and required or preferred in-services.
- Generates and develops ideas which improve the quality of care for patients in the facility.
- Performs all position duties according to safety rules and practices required for this position, and as required to ensure the general safety of staff, residents and visitors of this facility.
- Demonstrates knowledge and understanding of all policies and procedures and ability to reference them from appropriate manual.
- Exhibits the highest level of customer service towards residents, staff and visitors.
- Ability to work with the highest efficiency and time management.
- Exhibits flexibility and consistently demonstrates the ability to respond to changing situations to meet facility and patient needs.
