Summary:
We are seeking a RN with experience in home health to join our team. As a member of our home health team, our ideal candidate will be responsible for managing the home care of terminally ill elderly patients. We are looking for an individual with strong communication skills and dedication to providing patients and families with accessible, comprehensive care in their homes.
Responsibilities:
-Assess patients' condition and provide treatment according to their plan of care.
-Document patient condition and status changes accurately.
-Assist with ADLs as needed.
Pay: $37.00 - $42.00 per hour
