Fairview Rehabilitation & Healthcare is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis.
Working under the direction of a licensed Physical Therapist, you will help residents improve mobility, strength, balance, and overall functional independence. This is a great opportunity to join a supportive, interdisciplinary team focused on delivering high-quality, resident-centered care.

Implement physical therapy treatment plans developed by the supervising Physical Therapist

Assist residents with therapeutic exercises, gait training, balance activities, and mobility programs

Observe, document, and report resident progress and response to treatment

Collaborate with therapy, nursing, and medical staff to ensure coordinated care

Maintain professional relationships with residents, families, and team members
-
Deliver care in accordance with professional standards and facility policies

Active Illinois Physical Therapist Assistant (PTA) license or eligibility for licensure

Strong communication and interpersonal skills

Ability to work both independently and within a multidisciplinary team

Skilled nursing experience preferred but not required

Required Education:
- Graduate of an accredited Physical Therapist Assistant (PTA) program
- Current Illinois PTA licensure (or ability to obtain)
Required Experience:
- Experience in implementing individualized treatment plans in collaboration with a rehab team
- Experience in educating patients, families, and caregivers on treatment techniques
- Participation in patient care meetings and staffing conferences
Required Skills and Abilities:
- Excellent interpersonal and communication skills
- Strong critical thinking and problem-solving abilities
- Proficiency in therapy documentation systems (e.g., NetHealth preferred)
