Summit Medical Consultants is actively recruiting for a full-time Assistant Triage Nurse Manager (LPN) to join our team. The Assistant Triage Nurse Manager will be responsible for supporting the Triage Manager in oversight and accountability of the triage nurse and care management team as well as other administrative duties as assigned by the Triage Manager or members of the Leadership Team. This position will work under the supervision of the Triage Manager. This will be a salaried position.
Summit Medical Consultants is a rapidly growing Physician Practice. Founded in 2015, we have expanded to approximately 85+ employees and are continuing to grow. If we were a hospital in Colorado, we would rank 9th by number of admissions per year. Our mission is to provide patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care and Assisted and Independent Living stays. We coordinate with the Hospitalists and Specialist physicians and surgeons, Physical, Occupational, Speech Therapies, Case Management and Psychiatry at the facility level to provide compassionate and coordinated care.

Job responsibilities include, but are not limited to:
- Assistant Manager will be responsible for meeting expectations and target goals with Care Management programs monthly.
- Assistant Manager will be responsible for daily care management reporting.
- Assistant Manager will be responsible for weekly care management audits and reporting of audits to Triage Manager.
- Assistant Manager will be responsible for process improvements, practice initiatives, pilot projects, developing new workflows and goal driven solutions that support the strategic plan and vision of the organization for Care Management programs and Triage team.
- Assistant Manager will provide clinical and administrative oversight of the Triage Team as needed by the Triage Manager.
- Responsibilities include but are not limited to: training, mentoring, managing daily assignments, productivity of the care management team, providing back up for Triage Manager when assigned.
- Additional administrative duties as assigned.
- Accountability for meeting expectations and target goals with Care Management programs monthly.
- Accountability for performance of the Triage Team’s completion of daily tasks and workflows in accordance with current standard operating procedures and scope of practice.
- Participate in Admin on Call rotation
Department Expectations:
- Act as a communication hub between providers, facility partners, patients and their families and outside vendors.
- Address practice protocols in an appropriate manner.
- Document all patient, family and facility interactions. Including, but not limited to: prescriptions, verbal orders, clinical telephone calls.
- Assess the quality of patient care delivered by the Triage Team. Coordinate patient care services with patients, staff, providers and other outside sources as needed.
- Provide Case Management support and coordination related to Care Management programs.

Requirements:
● Active LPN Compact State nursing license
● 2 years clinical nursing experience in the post acute care setting
● Leadership experience preferred but not required

Schedule: Full time, 8 hour shift : Monday to Friday with rotating on call

Salary range $58,000-$62,000 depending on experience
