Job Summary
We are seeking an experienced and compassionate Advanced Practice Registered Nurse (APRN) to join our team. We are a primary care clinic specializing in serving Medicare Advantage members in Miami-Dade.
We’re looking for a practitioner who is skilled in providing high-quality, patient-centered care to older adults and who understands the importance of HEDIS measurements and quality metrics in managing our patient population.
Duties
- Conduct thorough patient assessments and diagnostic evaluations to determine health status and care needs.
- Develop and implement comprehensive care plans, including discharge planning and case management strategies.
- Perform suturing and other minor procedures as necessary, ensuring the highest standards of care.
- Provide education to patients and caregivers on chronic disease management and preventive health.
- Manage pain effectively through appropriate interventions and treatment plans.
- Provide occupational health services, focusing on preventive care and health promotion.
- Ensure high-quality documentation and adherence to HEDIS measures and other quality benchmarks.
- Engage in clinical research initiatives to enhance practice standards and patient care outcomes.
- Administer psychopharmacology treatments as needed, monitoring patient responses and adjusting plans accordingly.
- Collaborate with interdisciplinary teams to coordinate patient care and optimize treatment pathways.
Qualifications
- Active Florida APRN license.
- Master’s degree in nursing or a related field from an accredited institution.
- Strong clinical skills in areas such as pain management, diagnostic evaluation, and case management.
- Minimum of 2+ years of experience in primary care or geriatrics, preferably with Medicare populations.
- Experience in occupational health is preferred but not mandatory.
- Proficiency in performing sutures and managing complex patient cases.
- Bilingual (English/Spanish) a plus.
- Knowledge of psychopharmacology principles is advantageous.
- Excellent communication skills with the ability to work effectively within a team-oriented environment.
