A growing rehabilitation services organization is seeking a compassionate and experienced Part-Time Occupational Therapist (OT) or Physical Therapist (PT) to perform Durable Medical Equipment (DME) and Home Accessibility Evaluations throughout the community.
This position is ideal for a clinician who enjoys working independently while making a meaningful impact by helping individuals remain safe and independent in their homes. The successful candidate will conduct comprehensive assessments and provide clinical recommendations for mobility equipment, adaptive devices, and home modifications.

Responsibilities
- Conduct comprehensive in-home and community-based OT/PT evaluations.
- Assess functional mobility, transfers, positioning, and environmental safety.
- Evaluate clients for wheelchairs, seating systems, mobility devices, and other durable medical equipment.
- Recommend home accessibility modifications, including bathroom safety equipment, ramps, grab bars, transfer solutions, and environmental adaptations.
- Recommend adaptive equipment that promotes independence with activities of daily living.
- Prepare detailed clinical evaluation reports and supporting documentation.
- Collaborate with physicians, case managers, caregivers, vendors, and other healthcare professionals to coordinate appropriate recommendations.
- Educate clients and caregivers on equipment use and safety.
- Maintain accurate documentation in accordance with professional and regulatory standards.
- Stay current on DME products, home accessibility guidelines, and occupational therapy best practices.
Qualifications
- Active, unrestricted California Occupational Therapist (OT) or Physical Therapist (PT) license.
- Minimum one year of clinical OT or PT experience preferred.
- Experience performing DME evaluations, wheelchair assessments, home accessibility evaluations, or home health assessments strongly preferred.
- Knowledge of mobility equipment, positioning systems, adaptive equipment, and home modification recommendations.
- Excellent clinical reasoning and assessment skills.
- Strong written documentation abilities.
- Ability to work independently and manage a flexible schedule.
- Reliable transportation and willingness to travel locally for client visits.
- Current CPR certification preferred.
