Key Responsibilities of the Physical Therapist (PT):

- Valid PT license in Illinois.
- Current Illinois Driver’s License and reliable transportation for travel within the service area.
- Strong communication and documentation skills (Axxess experience preferred).
- home health experience.
- CPR certification.
