Flexible Hourly/PRN Position
About the Role
WellSpring provides in-home skilled nursing and supportive care services for older adults and medically complex clients. We are seeking dependable field nurses to provide scheduled in-home visits based on client needs.
This role is ideal for nurses looking for:

Responsibilities
- Vital signs, symptom assessment, and clinical documentation
- Caregiver and client education and coaching
- Skilled nursing services, including (but not limited to): Ostomy care, Foley catheter care, Feeding tube management and site care, Wound assessment and dressing changes, etc
- Medication reconciliation, education, and monitoring
- Chronic disease monitoring (CHF, COPD, diabetes, dementia, Parkinson’s)
- Post-discharge stabilization and readmission prevention
- Physician communication and care coordination
Qualifications
- Active Wisconsin RN license
- Strong clinical and assessment skills
- Comfortable working independently in clients’ homes
- Strong communication and documentation skills
- Home health, hospice, geriatrics, or skilled nursing experience preferred
Compensation & Benefits
