Duties
- Conduct comprehensive patient assessments to evaluate psychosocial needs, emotional well-being, and family dynamics.
- Develop individualized care plans incorporating counseling strategies such as mental health counseling, behavioral therapy, and motivational interviewing.
- Provide individual and group counseling sessions focused on grief, loss, coping skills, and adjustment to illness.
- Collaborate with interdisciplinary teams to coordinate hospice care services, including medical documentation and case management.
- Assist families with understanding medical terminology, navigating healthcare systems, and accessing community resources.
- Maintain accurate and confidential medical records in compliance with HIPAA regulations using Electronic Medical Record (EMR) systems.
- Offer crisis intervention and behavioral health support during urgent or complex situations involving patients or families.
Skills
- Proven experience in hospice care, home & community care, or inpatient settings with familiarity in post-acute care environments.
- Strong background in child & family counseling, behavioral health therapy, and clinical research methodologies.
- Knowledge of medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, and medical documentation standards.
- Expertise in working with individuals with disabilities, dementia care (including Alzheimer’s), autism spectrum disorder, PTSD care, and geriatrics.
- Skilled in crisis management, diagnostic evaluation, intake procedures, and working with diverse populations such as seniors or pediatric patients.
- Proficiency with EMR systems and data collection tools to track patient progress and clinical outcomes effectively.
- Excellent communication skills for conducting assessments like motivational interviewing and providing psychotherapy or behavioral therapy tailored to individual needs.
