Duties Include:
The registered nurse is expected provide direction to members of the patient care team through case management of a patient. Case management of patient services includes:
- Perform initial evaluation, follow up visit and discharge of your patient
- Design a care plan, and medical record baseline, and begin implementation of that care plan
- Communicate with other members of patient care team and adjust care plan as needed
- Reach out for community services to assist patient as needed
- Physical treatments with patients as per designed care plans
- Review notes/charts of LVNs and sign off on their notes, communicating with them regularly.
- Appropriate supervision of patients that have been handed to LVNs
- Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care
- Trains new clinicians by allowing clinicians to shadow patient visits
- Documents direct patient services, coordination and collaboration with physician and other disciplines
- Promotes the agency and services to the public and participate in physician and community education related to services provided by the agency as registered nurse.

Job Requirement
- Registered Nurse license to work in CA
- Minimum 1 year of Home Health experience
- OASIS Documentation experience is required to do SOC visits
- CPR / ALS Card
- Driver’s License or State ID Card
- Current Negative TB Test
- Current Physical Exam
- COVID Vaccine Card or Exemption Proof
- COVID Booster Proof
- Able to do Home Health Visits in Lancaster, Palmdale and nearby cities
