Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly
- Administer medications and perform catheter care following medical instructions and home healthcare regulations
- Support patients during transfers using patient transfer methods and equipment like Hoyer lifts or wheelchairs to ensure safety
- Maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring HIPAA compliance and confidentiality
- Provide companionship and emotional support to clients with dementia or Alzheimer's disease, fostering a safe and engaging environment
- Assist with light housekeeping tasks such as laundry, cleaning, meal preparation, and ensuring a clean living space for clients
- Follow all safety protocols including first aid, CPR certification requirements, and proper patient handling techniques to prevent injury
- Communicate effectively with clients’ families and healthcare teams to coordinate care plans and update medical records
Skills
- Proven experience working with geriatric patients or individuals with disabilities in settings such as nursing homes or assisted living communities
- Strong knowledge of medical terminology, medication administration procedures, and home healthcare regulations
- Skilled in patient observation, recognizing changes in condition or behavior that require escalation
- Familiarity with EMR/EHR systems for accurate documentation of patient care activities
- Ability to perform patient transfers safely using various methods and equipment like Hoyer lifts or transfer belts
- Excellent communication skills for engaging with clients’ families and healthcare professionals
- Knowledge of dementia care strategies and end-of-life support practices
- Capable of heavy lifting when necessary for patient transfers or mobility assistance
- Valid certifications in First Aid, Basic Life Support (BLS), CPR, and hospice or palliative care preferred
- Experience working within home health agencies or providing live-in caregiving services is highly valued
