Summary: The Guardianship Representative adds value to the Corporation by providing support to
assigned wards with medical and financial decisions. The Guardianship Representative may be hired
on either a full or part-time basis.
Essential Duties and Responsibilities: This list of duties and responsibilities is not all-inclusive and
may be expanded to include other duties and responsibilities as management may deem necessary
from time to time.
Service Delivery
? Safeguards the health as well as the physical, mental, and financial well-being of wards by
making informed, appropriate, and sound decisions regarding their care.
Establishes and maintains positive working relationships with service providers, social service
agencies, funding sources, family members, court employees, etc. by ensuring timely responses
to inquiries while overseeing the care and safety of the wards.
Participates in and provides direction for planning activities related to ward care, which includes
advocating for services, meeting the ward’s best interest, taking into consideration the safety
and personal preferences.
Conducts routine visits (monthly) to monitor the ward’s care and functioning in their residences,
day programs, or other settings, which includes addressing discovered issues promptly and
proactively.
Completes required legal documentation per State and county policies, which includes informing
relevant individuals of status changes, and attending and testifying at court hearings.
Maintains timely, ongoing case notes for individuals that are being supported, as well as
maintaining updated demographics.
Completes statutorily required continuing education credits (minimum of 10 hours per year).
Participates in assigned meetings, professional development, and training as required
Responds to emergency situations requiring decisions related to care, necessary notifications
and final arrangements.
Provides information and referral service to caregivers or community members.
Guardian of Person Responsibilities
Manages a caseload of agency wards, including but not limited to the duties listed below:
Explains the role of the guardian, the rights of the ward, and ward grievance rights and
filing processes.
Completes a comprehensive Guardianship plan within 10 days of Ward appointment
Photographs Ward
Notifies relevant agencies and individuals of the guardianship appointment and provides
appropriate documentation. Gains knowledge of ward’s medical status.
Ensures that the ward receives needed medical and financial services and is properly
cared for, provided for, has the opportunity to exercise legal rights.
Conducts face-to-face contact with his or her Ward monthly unless otherwise
directed by supervisor.
All voicemails and phone calls are returned within 24 hours. Friday calls MUST be
returned no later than Monday by noon.
Accurately completes Ward case notes within 24 hours of each contact and reviews
and submits billing notes by the 3rd day of each month.
Provides wards with information and referrals to other community resources as
appropriate.
Other Duties As Assigned
Provides coverage for Guardianship Representatives in their absence or as needed.
Completes Annual Report of Conditions on assigned wards upon request.
Performs all guardianship duties following procedures as adopted by the Board of Directors.
Performs all other duties as determined appropriate by the Program Manager and Guardianship
Representative Supervisor.

Education, experience and skills required:
The Guardianship Representative must have experience in Human Services or a related field,
or the equivalent combination of education and work experience.
Demonstrated ability to work independently with a minimum amount of supervision.
Experience completing complex paperwork and documentation, in a timely manner.
Demonstrated ability to represent agency in a professional manner to clients, partnering
agencies and in the community
Must demonstrate an ability to deal with guardianship clients in an effective & compassionate
manner
Demonstrated strong knowledge in the issues relating to guardianships and the related local,
county and state regulations.
An unmatched degree of integrity, with highly developed skills in fostering synergy, enthusiasm,

Travel between multiple sites is required.
Must have a valid driver’s license and reliable transportation.
Must have proof of insurance AND meet the requirements of the Employee Driving Policy.
Must pass a Wisconsin Caregiver background check, including criminal records check for WI
and previous states of residence.
Motor Vehicle Release (MRV) are required for this position.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk
or hear. The employee frequently is required to use hands to finger, handle, or touch objects. The
employee is occasionally required to stand; walk; sit; reach above shoulders; climb or balance; and
stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 25 pounds when
stocking office supplies.
Office Technology: Ability to utilize equipment, office software (i.e. EMS, Word, Excel) and web based
applications to meet the needs of staff, co-workers and ANS. Ability to utilize office equipment to
