We are seeking an experienced and compassionate Registered Nurse (RN) with hospice experience to join our team and provide care to patients throughout the Valley Area of California.
Position Summary
The Hospice Registered Nurse is responsible for assessing, planning, coordinating, and providing skilled nursing care to hospice patients in their homes, assisted living communities, skilled nursing facilities, and other care settings.
The ideal candidate has previous hospice experience, strong clinical judgment, excellent communication skills, and a genuine commitment to supporting patients and families during end-of-life care.
Essential Responsibilities
- Perform comprehensive nursing assessments for hospice patients and identify changes in clinical condition.
- Develop, implement, and update individualized patient care plans in collaboration with the interdisciplinary hospice team.
- Provide skilled nursing care focused on comfort, symptom management, pain control, and quality of life.
- Monitor patients for changes in condition and communicate significant findings to the physician, hospice medical director, and interdisciplinary team.
- Educate patients, family members, and caregivers regarding medications, symptom management, disease progression, safety, and end-of-life care.
- Administer medications and treatments according to physician orders and the established plan of care.
- Provide emotional support and guidance to patients and families throughout the hospice process.
- Coordinate patient care with physicians, social workers, chaplains, hospice aides, caregivers, pharmacies, facilities, and other members of the care team.
- Participate in interdisciplinary group/team meetings and contribute to ongoing care planning.
- Complete nursing documentation accurately and within required timeframes.
- Perform hospice admissions, routine visits, recertification assessments, and other nursing visits as assigned.
- Assist with management of urgent patient needs and changes in condition.
- Ensure patients have appropriate medications, supplies, durable medical equipment, and other resources required for comfort and safety.
- Provide caregiver education regarding signs and symptoms associated with end-of-life changes.
- Maintain communication with patients and families to ensure concerns are addressed promptly and compassionately.
- Follow hospice policies, infection-control procedures, professional nursing standards, and applicable regulatory requirements.
- Participate in quality improvement and patient care initiatives as needed.
- Travel throughout the assigned Valley service area to provide patient care.
Qualifications
- Current and unrestricted Registered Nurse (RN) license in the State of California.
- Previous hospice nursing experience strongly required.
- Experience providing end-of-life, palliative, home health, or similar patient-centered care is highly desirable.
- Strong clinical assessment and symptom-management skills.
- Knowledge of pain management and common end-of-life symptoms.
- Excellent communication, organizational, and documentation skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Compassionate, dependable, patient-focused, and comfortable supporting patients and families during sensitive situations.
- Ability to prioritize multiple patient needs and respond appropriately to changes in condition.
- Current CPR/BLS certification, as required for the position.
- Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.
- Basic computer skills and ability to complete electronic medical record documentation.
Preferred Experience
- Hospice RN experience.
- Hospice admissions and case management experience.
- Experience working with patients in private homes, assisted living facilities, and skilled nursing facilities.
- Familiarity with hospice electronic medical record systems.
- Experience educating families and caregivers regarding symptom management and end-of-life care.

Work Environment
This is a field-based hospice nursing position serving patients throughout the Valley Area, California. The RN may provide care in patients' homes, assisted living communities, skilled nursing facilities, residential care facilities, and other approved hospice care settings.
Travel within the assigned service area is required.
