All Care and Comcare Home Health is currently seeking a qualified Physical Therapist to join our dedicated healthcare team. This position offers a unique blend of administrative oversight and hands-on patient care, presenting an excellent opportunity for a PT looking to expand their professional scope.
Position: Field Physical Therapist
Key Responsibilities:

Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.

Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).

Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.

Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.

Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.
Qualifications:

Active license/registration with the Physical Therapy Board of California.

Graduate of an accredited Physical Therapy program.

Strong documentation and organizational skills.

Excellent communication and interpersonal abilities.

Willingness to travel to patient homes for evaluations and treatment.

Valid California Physical Therapy license.

Current CPR certification.

Valid California driver's license.

Current California auto insurance.

Completed physical examination, TB test, and required vaccines/titers.
