Position Summary
We are seeking a compassionate and dependable Home Health Aide who is passionate about providing high-quality, patient-centered care in the comfort of clients’ homes. In this role, you will be an essential member of the care team, helping patients maintain dignity, independence, and safety while supporting their physical and emotional well-being.
Key Responsibilities
Provides attentive and respectful personal care according to the Aide Service Plan and individualized patient needs.
Communicates clearly and professionally with patients, families, and care team members, including accurately reporting clinical information.
Observes, documents, and reports changes in patient condition, body function, or behavior in a timely manner to the nursing supervisor.
Measures and records vital signs including temperature, pulse, and respiration accurately.
Maintains a clean, safe, and healthy environment for patients while promoting personal safety.
Follows standard precautions and infection control protocols at all times.
Assists patients with safe mobility, including transferring, positioning, ambulation, and range-of-motion support using proper techniques.
Recognizes emergency situations and takes appropriate action in accordance with established procedures.
Respects patient privacy, confidentiality, and personal property at all times.
Completes all required documentation accurately and on schedule.
Participates in care planning, interdisciplinary team discussions, and quality improvement activities.
Maintains required training and in-service education per agency policy.
Follows all agency policies, procedures, and regulatory requirements.
Performs additional duties as assigned.
Qualifications
Completion of an approved home health aide training and competency program or meeting Indiana state competency requirements.
Strong interpersonal skills with a caring and empathetic approach to patient care.
Ability to read, write, and follow directions effectively.
Dependable, organized, and able to work independently in a home setting.
Ability to collaborate with nurses, therapists, and other healthcare professionals.
Physical Requirements
Ability to squat, bend, kneel, and twist regularly.
Ability to stand for extended periods and assist with patient mobility.
Ability to push or pull up to 180 pounds and lift or carry up to 25 pounds.
Ability to wear a respirator when required for patient care.
Adequate vision and hearing to safely perform patient care duties.
Reliable transportation to travel to patient homes.
