Position Summary
The Registered Nurse (RN) provides clinical oversight and nursing services to residents in a Community-Based Residential Facility (CBRF). The RN is responsible for ensuring quality resident care, maintaining compliance with Wisconsin Department of Health Services (DHS) regulations, supporting direct care staff, and coordinating healthcare services with physicians, pharmacies, home health agencies, hospice providers, hospitals, therapists, and families.
The RN promotes a safe, compassionate, resident-centered environment while supporting the mission and values of the organization.
Essential Duties and ResponsibilitiesResident Care
- Complete comprehensive nursing assessments upon admission and as needed.
- Develop, review, and update resident care plans based on individual needs.
- Monitor residents for changes in physical, mental, and emotional condition.
- Perform nursing assessments following falls, illnesses, injuries, or significant changes in condition.
- Provide nursing interventions within the RN scope of practice.
- Coordinate physician appointments and follow-up care.
- Review hospital discharge paperwork and ensure continuity of care.
- Participate in resident care conferences with families and interdisciplinary team members.
Medication Management
- Review physician orders for accuracy and implementation.
- Monitor medication effectiveness and adverse reactions.
- Ensure medication administration complies with Wisconsin CBRF regulations.
- Provide medication education to residents and staff as appropriate.
- Coordinate medication refills with pharmacies and providers.
- Conduct routine medication cart and medication room audits.
- Quarterly Psychotropic Reviews and Monthly PRN Psycotropic Reviews
Clinical Oversight
- Serve as the clinical resource for caregivers and management.
- Delegate nursing tasks in accordance with Wisconsin Nurse Practice Act.
- Provide clinical guidance regarding infections, wounds, diabetes management, falls, and chronic disease management.
- Ensure infection prevention and control practices are followed.
- Monitor vital signs, weights, laboratory results, and health trends.
Staff Education and Training
- Provide ongoing staff education on:
- Medication administration
- Infection control
- Documentation
- Resident rights
- Dementia care
- Diabetes management
- Emergency procedures
- Fall prevention
- Complete competency evaluations as required.
- Delagation 1-4 times a month for new staff
Regulatory Compliance
- Maintain compliance with Wisconsin DHS Chapter 83 regulations.
- Assist with state surveys and licensing inspections.
- Review incident and accident reports.
- Ensure documentation meets regulatory standards.
- Participate in quality assurance and performance improvement activities.
- Maintain confidentiality in accordance with HIPAA.
Care Coordination
- Communicate with physicians regarding resident status and new orders.
- Coordinate services with:
- Hospice
- Home Health
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Behavioral Health
- Pharmacies
- Laboratories
- Case Managers
- Notify families and legal representatives of significant changes in condition.
Documentation
- Complete accurate and timely nursing documentation.
- Maintain electronic medical records.
- Document assessments, physician communications, interventions, care plans, and resident progress.
- Review documentation completed by direct care staff for accuracy and completeness.
Emergency Response
- Assess residents following incidents.
- Initiate emergency protocols as needed.
- Coordinate emergency transfers to hospitals.
- Complete required incident documentation.
Qualifications
- Current Wisconsin Registered Nurse (RN) license in good standing.
- Current CPR/BLS certification.
- Minimum one year of nursing experience preferred.
- Experience in assisted living, CBRF, long-term care, behavioral health, or geriatrics a must!
- Knowledge of Wisconsin DHS Chapter 83 regulations preferred.
- Strong assessment, leadership, communication, and organizational skills.
- Valid Wisconsin driver's license and reliable transportation.
Physical Requirements
- Ability to lift up to 50 pounds with assistance.
- Ability to stand, walk, bend, reach, and transfer residents when necessary.
- Ability to respond quickly during emergencies.
Work Environment
- Community-Based Residential Facilities serving adults with varying physical, cognitive, behavioral, and mental health needs.
- Travel between multiple facility locations.
- On-call responsibilities may be required. Every other weekend, taking triage calls

Reports To: RN Administrator/Director of Nursing
Hours: 32 hours/week and every other weekend on call for triage. Holiday On-call Rotation
Pay: From $36.00 per hour
