Premier Home Health. is seeking a dedicated Medical Social Worker to provide compassionate psychosocial support to patients and their families. Responsibilities
- Conduct psychosocial assessments for patients and their families/caregivers.
- Develop and implement individualized social work care plans.
- Provide counseling and emotional support related to illness, stress, grief, and end-of-life care.
- Coordinate referrals to community resources and support services.
- Collaborate with physicians, nurses, and the interdisciplinary care team.
- Maintain accurate and timely clinical documentation.
- Serve as a liaison between patients, families, and community agencies.
- Participate in care planning meetings and quality improvement initiatives.
- Build and maintain relationships with community organizations and service providers.
- Perform other duties as assigned by the Clinical Supervisor or Nursing Supervisor.
Qualifications
- Master's or Doctoral degree in Social Work from a CSWE-accredited program.
- Meets California personnel qualifications and applicable state regulations.
- Minimum of 1 year of healthcare experience (Home Health experience preferred).
- Excellent communication, documentation, and organizational skills.
- Current CPR Certification.
- Valid driver's license, reliable transportation, and current auto insurance.
For Associate/Unlicensed Social Workers
- Must practice under the supervision of a Licensed Clinical Social Worker (LCSW) in accordance with California regulations.
- Must inform patients of unlicensed status before providing applicable services.
- Will receive required face-to-face supervision from a qualified LCSW.
