Job Overview
We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our multidisciplinary healthcare team. Responsibilities
- Conduct detailed physical examinations and health assessments across various age groups, including pediatric and geriatric patients.
- Prescribe medications in accordance with state regulations and collaborative practice agreements, ensuring compliance with DEA regulations.
- Manage acute and chronic conditions such as infectious diseases, pain management issues, gastrointestinal concerns, and behavioral health needs.
- Perform diagnostic evaluations utilizing EMR/EHR systems like Elation Health to document patient encounters accurately.
- Collaborate closely with physicians, specialists, nurses, and other healthcare professionals to develop comprehensive care plans.
- Provide patient education on disease prevention, medication administration, nutrition, and lifestyle modifications to promote optimal health outcomes.
Experience
- Proven experience working in diverse healthcare settings such as hospitals, outpatient clinics, urgent care centers, or nursing homes.
- Demonstrated proficiency with EMR/EHR systems for medical documentation and patient management.
- Strong background in managing chronic care management
- Experience working with children (pediatrics), seniors (geriatrics), or individuals with disabilities including developmental disabilities or dementia enhances your candidacy.
- Knowledge of CPT coding, ICD-9/10 coding systems for billing and documentation purposes is essential.

Pay: $90,000.00 - $125,000.00 per year
