Coordinates resources and collaborate with all stakeholders to ensure the daily operation of Reentry Services. Maintains a caseload of individuals that are needing services to reenter the community. Working with detention venders / volunteers inside the facility and while working with community partnerships and resources.
Direct Client Service
Respond to inmate communications in a timely manner
Provide assistance in writing a resume, creating a Kansas Workforce Center profile, and in conducting job searches
Meet with clients to discuss individual plans, living arrangements, or any barriers they may be having to reentry
Assist with gaining government identification
Identify and secure in-home services to maintain stabilization once client returns home
Familiarize clients with guidelines and procedures of reentry services
Treatment Planning
Identify services needs and develop treatment or referral services plan with clients
Perform functional assessment of client to determine client’s strengths and abilities; assist clients in achieving their short term goals.
Contact service providers to request and arrange services; coordinate and monitor services rendered
Provide informational packets to clients to help facilitate their mental health appointments
Confer with client caregivers, families, physicians, lawyers, community service providers, and other professionals throughout the County and to secure and coordinate services
Provide information packets to clients to maintain basic needs (housing, food, medical)
Provide outreach, screening, and engagement services to potential participants
Respond to the immediate needs of referrals
Assist with Mental Health Discharge Planner for the discharge of inpatient referrals and coordination of transition to the community
Provide outreach to court ordered consumers
Provide Targeted Case Management (TCM) for assigned consumers to aid in achieving goals identified in the individualized treatment plan.
Provide Community Psychiatric Supportive Treatment (CPST) for assigned consumers to achieve treatment goals; interventions include.
Provide Attendant Care (AC) for assigned consumers to achieve treatment goals; interventions include.
Statistics and Documentation
Prepare Reentry Plan including client and family participation on cases where needed
Collaborate in review, updates, and progress on goals with Reentry Plan
Complete Notes for Record, Precautions statements, and authorizations for release of records as required and/or recommended
Provide monthly Statistics
Provide Progress Reports on active clients and court progress
Provide Progress Reports on current and historical records
Minimum Qualifications: Bachelor’s degree in human services field, education, or a related field. Degree may be substituted for experience, with one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy. Pass background check, and motor vehicle screens. Must complete orientations provided by Sedgwick County and Sheriff’s Office.
Preferred Qualifications: One year of experience in a Community Mental Health Center.
