Patients First Home Care is looking for a Full-Time, Hybrid Remote, Weekend Intake Coordinator.
Available Schedules: Friday-Monday or Thursday-Sunday. Remote: Saturday & Sunday only.
Duties
Assist in the coordination of patient care, under the direction of the Director and Supervisor of our Intake Department
Answer phones, receive all intake information for paraprofessional cases, and enter the appropriate information in the computer.
Complete intake forms and appropriately route them, for admission approval. Assist in the scheduling of appropriately skilled person to fill the case, input scheduling information on the computer, and continue to assist in the coordination of the scheduling. Schedule replacement workers, as needed, and input scheduling information. Take telephone referral information. Perform other duties, as directed. Enter patient information into computer, check Medicare eligibility. Communicate with Case Managers, families, physician offices or facilities.
Experience in Home Health preferred .

- CEU on Elderly Abuse, Human Trafficking and Alzheimer's/Dementia·
- TB test results
