Responsibilities
- Provide weekly case management sessions and adjust frequency based on resident needs
- Complete assessments, develop service plans and track resident progress
- Maintain accurate and timely documentation that meets all licensing and contractual requirements
- Coordinate referrals to clinical services, community partners, vocational and educational resources
- Support residents in building life skills, independence and long term goals
- Ensure resident safety and uphold confidentiality at all times
- Participate in team meetings, trainings, case reviews and crisis response
- Collaborate with outside agencies, families and providers to support resident progress
- Support the overall program environment and act as a positive role model
Qualifications
- High School Diploma with three years experience in a residential or related program, or
- Associate’s Degree with two years experience, or
- Bachelor’s Degree with one year of human services experience
- Strong communication and documentation skills
- Ability to facilitate groups
- Ability to work independently and as part of a team
- Valid driver’s license, clean driving record and reliable transportation
