Job Description
The Veteran Housing Case Manager will:
· Facilitate the delivery of appropriate supportive services to Veterans and their families who are experiencing homelessness or at risk of becoming homeless.
· Conduct needs-based assessments, assist Veterans with connecting to public benefits, VA benefits, housing resources and other community resources as deemed appropriate.
· Create and implement housing stability plans, and work alongside a Case Manager Team Lead to ensure the Veteran household is receiving the services/supports they need.
· Assist the Veterans with identifying and applying for permanent housing.
· Act as an advocate for the Veterans with both landlords and other community partners.
Knowledge and Skills:
· Good written, oral and interpersonal communication skills
· Strong computer skills
· Strong time-management, prioritization and critical thinking skills
· Ability to work both independently and in a team-oriented, collaborative environment
· Fundamental understanding of Trauma informed care and a strengths-based service approach
· Ability to maintain accurate and detailed documentation regarding client progress and interactions
· Ability to be flexible and adaptive to unexpected challenges and changes
· Ability to present themselves professionally and maintain good working relationships with landlords and other community partners
Required Qualifications:
· High school diploma or GED
· Working knowledge of fundamental case management duties
· Able to take a rotating on-call phone in 1-week increments
· Clean motor vehicle record and background check
Preferred Qualifications:
· 1-2 years of case management experience with vulnerable populations
· Associate or Bachelor degree in Social Work, Social Sciences, Public Administration, Psychology or related field
· Veteran preferred
Compensation and Benefits:
· $21-23/hour -based off experience
· Paid training to become a Certified Case Manager (will receive a $0.50 pay increase upon completion)
