About Elderberry Health
- Elderberry Health is a Tennessee-based primary care provider organization caring for all ages patients. We
practice full-scope primary care in an outpatient setting.
Why this fellowship exists
- NP and PA programs prepare graduates well academically, but the transition to independent practice is
challenging — especially in rural primary care, where patients are complex and clinical autonomy expectations
arrive quickly. We built this fellowship to close that gap. Fellows spend a structured year embedded in our
practice, working alongside experienced clinicians, building a graduated patient panel under direct mentorship,
and developing real competency in a broad range of conditions.
What fellows do
- Carry a graduated patient panel, starting at reduced volume and ramping toward
independent practice over 12 months
- Practice alongside a dedicated supervising clinician with daily case review
- Attend periodic didactic sessions covering chronic disease management, geriatrics,
behavioral health, common procedures, and rural practice patterns
- Complete defined clinical competencies across our care settings
- Receive structured monthly feedback with formal evaluations at months 4, 8, and 12
Who we're looking for
- Recent graduates of accredited NP (FNP, AGPCNP, AGNP) or PA programs
- Tennessee licensure or eligibility by start date
- Genuine interest in primary care for rural communities
- Coachable, curious, and ready to work in a rural community settings
No prior clinical experience is required

The path to a full-time role
- Fellows who complete the program and demonstrate clinical readiness may be offered full-time clinical positions
at Elderberry Health.

- Duration: 12 months, full-time
