Essential Duties and Responsibilities:
Clinical Oversight & Client Care
- Ensure the health, safety, and welfare of all clients served.
- Provide hands-on nursing care as needed, including medication administration, weekly nursing assessments, wound care, and monitoring of vital signs.
- Complete and document regular skin assessments; promptly report changes to management and the Medical Director.
- Monitor, document, and ensure adherence to all medical protocols and treatment plans.
- Manage medication systems, including ordering, refills, and coordination with pharmacies for med rolls. Also included, oversight of MAR documentation.
- Responsible for med administration training.
- Monitor treatments, repositioning schedules, and medical needs for clients unable to perform these tasks independently.
- Participate in infection control, safety, and fire safety programs.
- Assist clients with medication management following state and facility guidelines.
- Assist with implementation of individualized treatment plan.
- Provide crisis intervention, first aid, and emergency response within scope of practice.
Client Program Coordination
- Support clients in achieving goals outlined in their treatment plans.
- Ensure consistent implementation of compensatory strategies and therapeutic interventions.
- Assists with the coordination of medical appointments, including scheduling, transportation, and follow-up documentation as needed.
- Maintain up-to-date records of all medical appointments and evaluations.
- Participate in rounds with medical director.
- Attend client doctors’ appointments as needed.
Administrative & Supervisory Duties
- Provide oversight and supervision to client coaches and support personnel.
- Give input and assist with monitoring and evaluating staff performance; provide coaching, discipline, and annual evaluations in collaboration with the Director.
- Assist in recruiting, hiring, orientation, and training qualified staff and contract employees.
- Assist with ensuring personnel files, certifications, and annual training records remain current and compliant with licensing standards.
- Maintain accurate medication and nursing related documentation. Complete required reports, assessments, and ensure timely submission of administrative paperwork.
- Maintain compliance with all state, federal, and licensing regulations.
- On call availability 24/7 for medical related issues.
Operational Support
- Assist the Director with payroll processing and oversight of the time clock system.
- Participate in quality enhancement initiatives and assist with quarterly and annual reports.
- Participate in safety committee meetings, interdisciplinary team meetings, and staff development sessions.
- Implement and monitor compliance with all facility policies and procedures.
Professional Standards
- Serve as a positive and professional role model for staff and clients.
- Maintain confidentiality and uphold ethical standards in all client and personnel interactions.
- Communicate effectively with clients, families, staff, and external agencies to ensure continuity of care.
- Attend ongoing in-service training as required by state and company policy.
- Demonstrate flexibility in scheduling to meet the demands of a 24/7 program environment.
- Perform additional duties as assigned by the Director or organizational leadership.
Qualifications:
- Current LPN license in good standing with the State Board of Nursing.
- Minimum of two years of nursing experience; experience in rehabilitation, behavioral health, or residential settings preferred.
- Strong organizational, communication, and leadership skills.
- Proficiency in medical documentation and electronic health record systems.
- Ability to manage multiple priorities in a fast-paced environment.
- Current CPR and First Aid certification required.
Pay: $23.96 - $28.85 per hour
