Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. This vital role involves providing high-quality patient care across various settings, including long-term care, assisted living, and acute care environments. As an LPN, you will play a key role in supporting patient recovery, managing medical procedures, and ensuring comfort and safety for all individuals under your care. Responsibilities
- Deliver comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using EMR (Electronic Medical Records) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and other treatments following established protocols while ensuring compliance with HIPAA regulations.
- Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques.
- Monitor patients’ conditions closely, manage acute pain and infectious disease symptoms, and respond promptly to urgent needs in emergency or ICU settings.
- Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or hospice care.
- Maintain accurate medical records, document medical documentation thoroughly, and ensure adherence to ICD-10/ICD-9 coding standards for billing and reporting purposes.
- Support specialized care areas including dementia/memory care, Alzheimer’s care, pediatric or neonatal units, and behavioral health environments.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) in the state of employment.
- Proven experience working with diverse patient populations across settings such as long-term care facilities, hospitals (including Level I Trauma Centers), or outpatient clinics.
- Familiarity with EMR/EHR systems like Epic, Athenahealth, or eClinicalWorks; proficiency in medical terminology and physiology knowledge.
- Experience with critical care procedures including ventilator management, airway management, PICU or neonatal ventilator support preferred.
- Ability to perform venipuncture, phlebotomy, medication administration via multiple routes (including injections), and basic life support certification.
- Strong assessment skills with the ability to recognize acute changes in patient condition quickly.
- Excellent communication skills to work effectively with patients with disabilities or developmental challenges and coordinate with interdisciplinary teams.
