Service Area:
Tulsa and surrounding communities
Responsibilities:
- Conduct in-home patient visits
- Assess, diagnose, and treat acute and chronic conditions
- Manage medications and treatment plans
- Coordinate care with specialists, caregivers, and family members
- Maintain accurate documentation within our EMR system
Qualifications:
- Current Oklahoma Nurse Practitioner License
- DEA Certification
- Reliable transportation
- Excellent clinical and communication skills
- Home health, geriatrics, or primary care experience preferred
