Duties
- Assist clients with personal care tasks such as bathing, grooming, dressing, and toileting to ensure comfort and dignity.
- Support residents with mobility and transfers using manual handling techniques and equipment like Hoyer lifts or patient transfer methods.
- Monitor patients’ health by taking vital signs, observing for changes, and reporting findings in accordance with HIPAA regulations and care plans.
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary, ensuring safety during meal times.
- Help clients with medication administration following prescribed schedules and safety protocols.
- Maintain a clean, safe environment by performing light housekeeping duties including cleaning personal spaces and common areas.
- Provide transportation assistance for appointments or community outings, ensuring safety during travel.
- Document care activities accurately and communicate effectively
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly valued.
- Demonstrated ability to observe patients closely, monitor vital signs, and recognize signs of distress or changes in condition.
- Experience working with individuals with dementia
- Knowledge of care plans, medication administration procedures, and patient safety protocols such as CPR and first aid certification is essential.
- Familiarity with patient transfer methods including Hoyer lifts or other assistive devices enhances your effectiveness in this role.
- Strong communication skills for client interaction and collaboration within multidisciplinary teams are crucial.
- Background in home & community care or long-term care settings supports your success in providing consistent, compassionate support.
