Contract role at Sweden Valley Manor (HCF Inc.)
Position Summary: Registered Dietician assures high quality nutritional care for residents by maintaining standards of clinical practices.
Position Responsibilities for a Clinical Nutrition Associate:
* Follow all HCF policies and procedures.
* Discuss problems concerning residents at high nutrition risk with Registered Dietitian
* Complete medical nutrition therapy reviews, progress notes and develop/update plans of care for new admissions, readmissions, and annually for all residents.
* Complete MDS assessments accurately in accordance with schedule
* Evaluate nutritional status, document status in a progress note, update and evaluate care plans for each resident on a quarterly basis and PRN.
* Monitor weight records weekly and identify and document information concerning significant weight changes.
* Document monthly and PRN on residents who have ongoing pressure injuries.
* Attend Resident Care, Nutrition at Risk, and QAPI meetings when requested.
* Communicate resident assessed needs, dislikes, and nutrition interventions to the
Nutrition Services Supervisor
* Implement care plans as defined by interdisciplinary team. Write progress notes based on care plan needs.
* Conduct quarterly QAPI audits.
* Complete RD referral forms if applicable.
Supervisory Responsibilities Clinical Registered Dietitian:
* Routinely interact with the Nutrition Services Supervisor concerning food preparation and service operations for residents
* Confer with Corporate or Senior Dietitian regarding resident nutritional care.
* Assist the Nutrition Services Supervisor in setting up yearly in-services as needed.
* Assume operational duties in the absence of the Nutrition Services Supervisor as needed.
* Assist the Nutrition Services Supervisor in monitoring residents’ needs and care.
* Completes QAPI audits as requested.
* Input resident data into the computer as needed.
Knowledge, Skills and Abilities: Graduate of a 2-year Dietetic Technician or a 4 year Dietetic program, and eligible to become registered or licensed, or is Registered, Licensed Dietitian. Must be mentally alert, and able to read, write, and understand English.
Must understand geriatric resident requirements and special needs.
Must have planning/organizational abilities, and basic computer skills. Must have sound knowledge of all facets of the dietary department and be able to prepare and present in-service programs.
