American Traveler is seeking an experienced RN - Case Manager for a hospital-based position in California, requiring an active CA RN license and a minimum of 2 years of case management experience.

Hospital acute care setting, Case Management unit

Day shift schedule, Monday through Friday, 8:00 AM – 4:30 PM (5x8-hour shifts)

Focuses on utilization management, resource management, discharge planning, and post-acute care referrals across the care continuum

Manages complex acute patient populations with a goal of preventing unnecessary admissions and readmissions

Provides discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department

13-week contract

Active California (CA) RN license required

Current BLS certification required

Minimum 2 years of RN experience required

Coordinates integrated and coordinated care to promote patient wellness, improved outcomes, efficient utilization of health services, and minimized payment denials

Candidates living within 50 miles of the facility must contact their recruiter regarding local rate eligibility

Former employees or travelers must have been separated from the facility for at least 1 year before returning as a traveler

Only 1 holiday RTO request will be approved during the contract
