Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in Skilled Nursing Facility and long-term environment. The LVN will work collaboratively with healthcare professionals to ensure optimal patient outcomes while adhering to best practices and regulatory standards.
Duties
- Deliver direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to evaluate patient conditions.
- Assist in the development and implementation of individualized care plans.
- Provide support for patients with chronic illnesses, Alzheimer’s disease, dementia, and developmental disabilities.
- Perform catheter care and wound management as needed.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
- Educate patients and families on health management, medication administration, and post-care instructions.
- Participate in quality improvement initiatives and infection control practices.
Experience
Candidates should possess experience in the following area:
- Long-term care or nursing home experience
Preferred skills include familiarity with electronic medical records (EMR) systems such as PointClickCare. Basic life support (BLS), medication administration protocols, and patient assessment procedures is essential. Experience with specialized nursing skills such as tube feeding, specimen collection/processing, and pain management is highly desirable. Pay: $33.00 - $40.00 per hour
