Alliance is looking for a part-time, qualified PHN to join our team. This position offers a highly flexible schedule with the ability to set your own visit days and times. Our full-time PHNs conduct around 15 visits per week. Visits will be completed primarily in Dakota and Washington counties, but could require the possibility of traveling to Anoka and Ramsey County. PHNs receive their referrals on a monthly basis, allowing ample time to schedule, conduct, and return visit documentation in a timely manner. The rate of pay starts at $65.00 per assessment (depending on qualifications and experience) in addition to a designated rate for any administrative time. Mileage and cell phone reimbursement is also included.
Responsibilities:
· Meet clients in their home to complete medical assessments for services
· Determine need for programming (CFSS, PCA, Home Health Aide, Homemaking, etc.)
· Work with a variety of individuals from different ethnic, religious, cultural backgrounds – use interpreters as needed
· Complete assessment documentation within the required time period
· Other duties as assigned
Qualifications:
· Driver's license, clean driving record, auto insurance, and reliable vehicle
· Willingness to travel throughout the area
· Ability to pass a DHS background check
· Ability to work independently
· Excellent communication skills
Experience completing MnChoices or Legacy Assessments
· Provide proof of a current PHN certificate*If you don't currently have your certificate and you meet the below qualifications you can obtain your certificate which is required for this position:
· RN licensed and currently registered in Minnesota,
· Baccalaureate or higher degree with a major in nursing,
· Completed course work which included theory and clinical practice in public health nursing (Details are available in MN Rule part 6316.0100 subpart 4.
