We are a Visiting Doctors Group seeking a compassionate, qualified Nurse Practitioner to conduct telehealth assessments for our home care patient population. Remote-friendly role, though occasional in-person visits may be available.
Responsibilities:
- Conduct telehealth visits/assessments for homebound patients
- Evaluate, diagnose, and develop/update treatment plans
- Document visits accurately and coordinate with our physician/clinical team
- Communicate effectively with patients who may have complex or chronic conditions
Requirements:
- Active NP license in New York State (required)
- Active PECOS enrollment (required)
- Prior home care/home health experience required
- Proper, thorough documentation skills
- Consistent, reliable, professional
- Patience/compassion with elderly and special needs populations
- Comfortable with telehealth platforms
- Strong communication skills
Compensation: Per visit, $20–$30 depending on complexity
Schedule: Minimum 30 hours/week required. Could look like 3, 4, or more days per week, as long as it adds up to at least 30 hours weekly.
