We are seeking an experienced Home Care Marketer / Veteran Liaison to drive client enrollment across our seven-county territory: Blair, Cambria, Clearfield, Indiana, Somerset, Bedford, and Westmoreland Counties. The ideal candidate will establish consistent referral pipelines for non-medical home maker, HHA and in-home respite care services by building relationships with VA Medical Centers and outpatient clinics. You will primarily coordinate with the VA Medical Center in Altoona and the VA Pittsburgh Healthcare System. While veterans are our primary focus, you will also identify and secure private-pay clients throughout the territory to diversify our active caseload.

- This position features a highly competitive base salary paired with an aggressive, uncapped monthly commission structure tied directly to the recurring monthly revenue of the clients (both VA and Private Pay) you personally enroll and manage. Key Responsibilities
- Build VA Referral Pipelines: Network directly with VA discharge planners, case managers, and social workers at regional VAMCs and local Community Based Outpatient Clinics (CBOCs) to secure veteran client referrals.
- Manage VACCN Authorizations: Navigate the Optum Provider Portal and Health Share Referral Manager (HSRM) to monitor, secure, and extend active Standardized Episodes of Care (SEOC) authorizations for Region 1.
- Drive Private-Pay Growth: Cultivate relationships with local elder law attorneys, trust officers, hospital discharge planners, and private care managers to capture self-pay clientele.
- Local Community Outreach: Conduct educational presentations at VFW posts, American Legion halls, senior centers, and county veteran affairs offices across West-Central Pennsylvania.
- Guide Veteran Families: Educate families on how to navigate the VA CCN assessment process to maximize their approved care hours for homemaker-home health aide services.
- Drive Strategic Growth: Meet monthly referral goals and document all sales activities within our company CRM.
Requirements
- Experience: Minimum 2 years of professional marketing experience in non-medical home care or home health.
- VACCN Expertise: Complete familiarity with Optum Region 1 billing paths, VA Community Care guidelines, and third-party administrator protocols.
- Local Network: Existing, trusted relationships with social workers and care coordinators within the regional Pennsylvania VA healthcare networks is highly preferred.
- Mobility: Willingness to travel daily across the region (Company vehicle provided). Valid driver's license with a clean driving record is mandatory.
- Skills: Self-motivated closer, empathetic communicator, and professional presentation skills.
