Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to provide comprehensive home health care services. The RN Home Health will deliver personalized patient care, coordinate treatment plans, and support patients in their recovery and ongoing health management within their homes. This role requires a highly skilled professional with experience across various medical settings, including acute care, geriatrics, pediatrics, and chronic disease management. The ideal candidate will possess strong clinical skills, familiarity with electronic health records, and the ability to work independently while collaborating with multidisciplinary teams to ensure optimal patient outcomes.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health history documentation using DSL/datasoft logic
- Complete SOC , ROC and Discharge OASIS.
- Manage complex medical procedures such as wound care, ostomy care, catheterization, tube feeding.
- Provide specialized care for patients with chronic conditions such as dementia, Alzheimer’s disease, memory care needs, or developmental disabilities.
- Educate patients and families on disease management, medication administration, nutrition, pain management, and health coaching strategies.
- Monitor patient status through ongoing assessment of vital signs and physiology knowledge to detect changes requiring escalation or urgent intervention.
- Adhere to infection control protocols and sterile processing procedures to prevent healthcare-associated infections.
Experience
- Proven experience in home health nursing with a background in medical-surgical nursing or outpatient clinics
- Demonstrated ability to work independently in a home setting while maintaining high standards of patient safety and quality of care.
- Experience in completion of OASIS preferred.
Pay: $80.00 - $180.00 per hour
