Position: On-Call Hospice Registered Nurse (RN)

Schedule: Weekends: Saturday 8:30 a.m.- Monday 8:30 a.m.
Position Summary
Harborside Hospice is seeking a compassionate, experienced Registered Nurse to join our hospice team as an On-Call Hospice RN. The On-Call RN provides after-hours nursing care, triages patient concerns, responds to urgent visits, and supports patients and families with compassionate end-of-life care in homes, assisted living facilities and nursing facilities.
This role requires excellent clinical judgment, strong communication skills, and the ability to work independently while collaborating closely with the interdisciplinary hospice team.
Responsibilities
- Respond to after-hours telephone calls from patients, families, caregivers, and referral sources.
- Perform urgent and emergency hospice visits as needed.
- Admit appropriate individuals to hospice care as requested during after-hour periods.
- Assess patients' physical, emotional, and psychosocial needs.
- Manage pain and symptom control according to hospice standards and physician orders.
- Provide patient and caregiver education regarding disease progression, medications, and comfort measures.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, and other members of the interdisciplinary team.
- Accurately complete electronic documentation in a timely manner.
- Maintain compliance with all federal, state, and hospice regulatory requirements.
Qualifications
- Current RN license in MA & NH
- Minimum of two years of nursing experience required.
- Hospice, long-term care or palliative care experience preferred.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Strong assessment, communication, and critical-thinking skills.
- Ability to work independently and make sound clinical decisions in the field.
- Experience with electronic medical records.
