The Hospice Registered Nurse (RN) Case Manager is responsible for providing comprehensive nursing care and case management services to an assigned caseload of hospice patients. This role combines direct patient care with care coordination, documentation, interdisciplinary collaboration, and administrative responsibilities to ensure patients receive high-quality, compassionate end-of-life care.
Working both in the field and collaboratively with the interdisciplinary team, the RN Case Manager serves as the primary clinical coordinator for their patients, ensuring continuity of care, regulatory compliance, timely documentation, and exceptional patient and family experiences throughout the hospice journey.

The ideal candidate is organized, communicates effectively, takes ownership of their caseload, and values working collaboratively with an interdisciplinary team to provide exceptional end-of-life care.

Manage and coordinate a caseload of hospice patients throughout the continuum of care.

Complete comprehensive patient assessments and develop individualized plans of care.

Collaborate with physicians, nurse practitioners, hospice aides, social workers, chaplains, volunteers, and other members of the interdisciplinary team.

Monitor patient conditions and recommend appropriate changes to the plan of care.

Provide symptom management and end-of-life nursing interventions.

Educate patients and families regarding disease progression, medications, comfort measures, and available hospice services.

Complete accurate and timely clinical documentation in accordance with Medicare, state, and agency requirements.

Participate in Interdisciplinary Group (IDG) meetings and case conferences.

Supervise Hospice Aides and LPNs in accordance with regulatory requirements.

Perform admissions, routine visits, PRN visits, recertification visits, and discharge visits as assigned.

Participate in the agency's on-call rotation.

Serve as a compassionate advocate for patients and families while maintaining the highest standards of clinical excellence.

8:00 AM-5:00 PM
Participation in the on-call rotation is required.

$150 per weekday on-call (Monday – Thursday)

$450 for weekend on-call (Friday – Sunday)

Graduate of an accredited School of Nursing.

Current Georgia Registered Nurse license in good standing.

Minimum one year of RN experience required.

Hospice or home health experience preferred.

Current CPR certification.

Valid driver's license, reliable transportation, and automobile insurance.

Strong assessment, critical thinking, communication, and documentation skills.

Working knowledge of Medicare Conditions of Participation and hospice regulations preferred.
