Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health status by observing vital signs and overall condition, reporting any concerns promptly
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding as necessary
- Support medication administration following care plans and HIPAA privacy standards to ensure confidentiality
- Maintain a clean and safe environment through light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Use specialized equipment like Hoyer lifts or resident lifts to safely transfer clients with mobility challenges
- Document care activities accurately in compliance with healthcare regulations and assist in updating personalized care plans
Requirements
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Knowledge of patient observation, dementia care (including Alzheimer’s and memory care), and working with individuals with disabilities or developmental disabilities
- Certifications such as CPR, First Aid, and HIPAA compliance are strongly desired; willingness to obtain necessary certifications if not already held
- Ability to perform heavy lifting and physical tasks required for resident transfers or mobility support
- Excellent communication skills for working effectively with clients, families, and healthcare teams
- Compassionate attitude with a strong commitment to providing respectful, patient-centered care
- Valid driver’s license and reliable transportation are a plus for home visits

Pay: $16.00 - $18.50 per hour
Work Location: In person
