Position Overview:
The CESA is responsible for providing comprehensive case management services to all assigned clients. This includes conducting assessments, developing, and implementing individualized care plans, coordinating services, advocating for clients' needs, and maintaining accurate and up-to-date documentation. The CESA will work collaboratively with other professionals and community resources to ensure that clients receive the highest quality of care and support.
Position Responsibilities
Provide Individualized Support: Conduct comprehensive assessments to identify clients' needs, strengths, and goals. Develop individualized service plans based on their unique circumstances and collaborate with clients to establish achievable objectives. Ensures a harm reduction approach while leveraging motivational interviewing skills to support the person served.
Community Integration: Assist clients in accessing and utilizing community resources, services, and supports to enhance their quality of life. Facilitate connections with housing, employment, education, healthcare, and other community-based programs to address factors related to social determinants of health
Skill-Building: Teach and support individuals in developing essential life skills such as problem-solving, decision-making, budgeting, time management, and socialization. Offer guidance and encouragement to empower clients in managing daily activities and maintaining stability.
Crisis Intervention: Respond promptly and effectively to crisis situations, providing appropriate support, de-escalation techniques, and linkage to emergency services when necessary. Collaborate with emergency responders, mental health professionals, and other relevant parties to ensure the safety and well-being of clients. Advocacy: Serve as an advocate for clients' rights, ensuring their voices are heard and respected. Collaborate with other service providers, community organizations, and stakeholders to advocate for improved mental health services and support systems.
Documentation and Reporting: Maintain accurate and up-to-date records of client progress, service plans, interventions, and outcomes. Complete required documentation in a timely manner according to Front Steps policy, CARF, and OhioMHAS standards while adhering to confidentiality and data privacy guidelines.
Collaborative Teamwork: Work closely with a multidisciplinary team of internal and external professionals, including psychiatrists, therapists, case managers, and peer support specialists. Participate in team meetings, case consultations, and coordination of care to ensure a holistic approach to client care.
Work Characteristics:
- Strong understanding of Medicaid, OHMHAS, Fair Housing, HIPAA, CARF and applicable
regulations, standards, and guidelines.
- Excellent organizational and time management skills.
- Ability to maintain accurate and up-to-date documentation.
- Working knowledge of Electronic Health Care Record portals.
- Strong attention to detail and accuracy.
- Knowledge of community resources and service options.
- Excellent communication and interpersonal skills with clients, colleagues, and external
partners.
- Ability to work independently and as part of a team.
Working Conditions:
- Work schedule: M-F daytime schedule.
- Work environment: Hybrid remote/on site.
- While performing the duties of this job, the employee is occasionally required to stand, climb
stairs, balance, sit, walk, type, kneel, and crouch.
- The employee must occasionally lift and/or move up to 25 pounds.
Minimum Qualifications:
- Associate degree in human services field required (bachelor’s degree in a related field
preferred).
- Current state licensure or certification as a social worker, counselor, or related professional.
LSW, CDCA, or comparable behavioral health credential.
- Minimum of 2 years of experience working with people with disabilities in a social service
setting.
- Valid driver's license and reliable transportation for local travel as needed.
