Mutual Assistance Network (MAN) is a community development organization dedicated to advancing social and economic opportunities so families can thrive. POSITION SUMMARY:
The Home Visitor shall serve as the family’s primary contact with the Birth & Beyond (B&B) program. Under the supervision of the Team Leader, the Home Visitor is responsible for developing a relationship with the family and working in their home and community to build upon strengths and to enhance self-sufficiency. The Home Visitor will be tasked with providing culturally responsive services and referral/resources to enhance the social and emotional development of families with children 0-18 years. The Home Visitor acts as a communication and trust bridge between the family, child protective services, social services and mental health systems. All duties will be performed within a strength-based, culturally responsive and in a trauma informed manner.
This position is non-exempt, hourly and full time.
Compensation: Non-exempt, $23.00 - $25.00/Hourly
ESSENTIAL FUNCTIONS:
1. Home Visitation
- Conduct regularly scheduled home visits to assigned families, to provide the Nurturing Parenting Program to parents and children in their homes.
- Conduct screens and assessments on all assigned families.
- Work with families to empower them to engage in problem solving.
- Model good parenting behavior, provides support, education/ information, and provides parents with age appropriate parenting and positive discipline techniques.
- Promote parent participation in home activities (lesson) designed to promote the physical, emotional, social, and intellectual development of their children.
- Work with families to develop, review, and update Family Nurturing Plans per Birth & Beyond Policies & Procedures under the direction of the Team Leader.
- Model nurturing attitudes and behaviors in all contacts with program participants and program staff.
- Utilize community resources, in-home teaching, and supportive problem solving to help families meet the goals of their nurturing plans.
- Advocate for and act as a liaison for families with existing community services and agencies, which may include routine translation and interpretation.
2. Supervision, Multi-Disciplinary Resource Team Case Reviews, Training
- Attend weekly supervision with the Team Leader.
- Participate as a member of the B&B Multi-Disciplinary Resource Team (MRT) of service providers, presenting assigned families to the MRT as required.
- Consult with MRT members regarding issues the family may experience that need additional assessments, referrals, or services.
- Facilitate and document ongoing progress of the MRT action plan.
- Attend required B&B trainings provided by The Child Abuse Prevention Center, the County of Sacramento, and its Contractors.
3. Case File Documentation
- Complete documentation as required by B&B Policies & Procedures; including but not limited to: Face to Face reports; Family Nurturing Plan; Multi-Disciplinary Resource Team forms; Immunization Records; Critical Incident Reports, CPS reports, etc.
- Assist and submit accurate and timely reports, progress notes, and data collection
4. Other Functions:
- Assist with and participate in outreach, community engagements, referrals, and volunteer generation.
- Maintain a safe, clean, resourceful, and enriching environment for families and staff.
- Assist with, conduct and/or participate in Mutual Assistance Network’s Community Centers (also referred to as Family Resource Center) classes, events and activities. Assist as needed with play care.
- Participate in health insurance outreach and Medi-Cal Administrative Activities (MAA) claimable activities as defined in the MAA claim plan, and accurately complete MAA time studies.
- Perform other duties as assigned that are associated with the B&B Performance Measures.
- Must report to work site as scheduled and agreed upon with Supervisor (Team Leader)

- Knowledgeable about local community resources.

- Strong interpersonal skills and the ability to relate to individuals who may not share basic commonality, including value systems and behavior norms.
- Experience in working with culturally diverse communities and families, with the ability to be culturally sensitive and appropriate.
- Demonstrate good writing and organizational skill.
- Skill to maintain a professional, confidential work environment.
- Ability to manage multiple tasks in an efficient manner.
- Knowledge of child abuse issues, substance abuse and family dynamics.
- Knowledge of child development and behavior.
- Ability to work evenings and weekends, as required.
- Ability to communicate clearly both orally and in writing.
- Experience in social services in community settings with families preferred.
- Able to deal with stressful situations.
Community Engagement and Responsiveness:
- Provide care to reduce barriers and ensure effective support for all members of the community.
- Demonstrate strong communication and collaboration skills to engage with residents in North Sacramento, particularly Del Paso Heights and Arden Arcade, and serve families with professionalism, respect, and sensitivity.
- Commit to ongoing learning and adaptation to better serve the needs of the local population.
- Maintain familiarity with the community and neighborhood where the Family Resource Center is located, as well as surrounding areas.
- Possess or demonstrate a willingness to develop knowledge of community values, beliefs, and practices.
- Show a commitment to continuous improvement in serving and engaging with the community effectively.
Physical Qualifications:
- Ability to work in office setting and operate basic office equipment (phones, fax and copy machine)
- Ability to perform basic computer functions (word processing, internet research, email
- Ability to work with individuals in their home environment
- Must be able to lift and carry up to 15 lbs.
- Ability to frequently sit, bend, twist, squat, kneel, reach, push and pull
- Ability to occasionally walk up to one mile intermittingly with breaks
- Ability to climb stairs
Other Qualifications:
- Ability to read, write and fluently speak Spanish.
- Must be at least 18 years of age.
- Must be a U.S. citizen or lawful permanent resident.
- Clearance of fingerprint background check.
- Must possess a high school diploma or GED.
- Must possess a valid California Driver’s license, reliable transportation and auto insurance.
- Able to travel between sites and to offsite events.
