Case Manager (Up to 4 positions)
(One year only)
Adult Education- Various Adult Education Schools
Licensed, Appointed and Tenured New York City Public Schools (NYCPS) Teacher. Preference may be given to appointed teachers currently working for D79-Adult Education.
- Satisfactory ratings
- Experience or willingness to work with adult students
- Demonstrate ability to design and implement on-site support for students
- Demonstrate proficiency utilizing Microsoft programs
- Demonstrate success in working collaboratively with professional staff
- Demonstrate ability to maintain effective relationships and open communication with students
- Excellent oral and written communications and interpersonal skills
- Commitment to continuous Professional Learning
- Highly organized
- Satisfactory attendance and punctuality for three years
Case Manager will work with students to remove barriers to retention and educational advancement. Responsibilities will include:
- Supporting students’ education plan assessment and retention
- Promoting Social Emotional and Academic Learning support
- Assisting students with college & career exploration including student interests, job search, interviews, and resume writing
- Meeting with students to assess their needs, help them identify long-term goals and address existing and potential barriers
- Helping students evolve from one state of development to the next, focusing on education and employment
- Conducting regularly scheduled follow-up on the outcomes of referrals and notify program of outcomes
- Using city and state online tools, to include ACCESS NYC, NYSDOL and NYS’s MY Benefits, to support students and/or to increase student access to benefits and supports
- Maintaining relationships with referral partners and providing coordination of partnership referrals by following the process prescribed by each partner.
- Scheduling and/or conducting workshops based on the needs of students.
As per UFT Collective Bargaining Agreement of 30 hours and 50 minutes a week based on the program hours of 9:00 AM – 9:30 PM, including two evenings and Saturdays
Three-year school term, subject to budget availability and satisfactory performance
