We are seeking a dynamic and experienced Healthcare Marketing Liaison who is confident, outgoing, organized, and passionate about building relationships to join our team and serve as a key connection between our skilled nursing and rehabilitation center, hospitals, physicians, healthcare professionals, residents, families, and community partners.
The ideal candidate is an energetic relationship builder with experience in skilled nursing, rehabilitation, long-term care, hospital discharge planning, healthcare admissions, or post-acute care.
This is a highly visible, community-facing position for someone who understands the healthcare referral process and knows how to develop meaningful relationships that support census growth and successful transitions of care.
Develop and maintain strong professional relationships with hospital case managers, discharge planners, social workers, physicians, home health agencies, hospice providers, assisted living communities, and other healthcare referral sources.
Identify new referral opportunities and actively expand the facility's referral network.
Conduct regular hospital, physician office, and community outreach.
Represent the facility at healthcare networking events, community events, professional meetings, and educational programs.
Maintain consistent communication and follow-up with referral partners.
Promote the facility's short-term rehabilitation, skilled nursing, long-term care, and specialized services to appropriate referral sources.
Respond promptly to incoming referrals from hospitals, healthcare agencies, physicians, residents, and families.
Serve as a liaison between referral sources, families, and the facility's admissions and clinical teams.
Assist with gathering information needed to facilitate timely admissions.
Coordinate with the interdisciplinary team to help move referrals through the admissions process efficiently.
Communicate admission requirements, services, and placement options to prospective residents and their families.
Support the readmission process when needed.
Help identify barriers to admission and work collaboratively with the appropriate departments to resolve them.
Maintain a strong focus on quality referrals, timely admissions, and census development.
Develop creative and effective marketing and outreach strategies to increase awareness of the facility.
Coordinate facility tours and meet with prospective residents, families, and referral partners.
Participate in community education, health fairs, senior events, networking opportunities, and professional organizations.
Develop and maintain positive public relations within the local healthcare community.
Communicate the facility's strengths, clinical capabilities, rehabilitation services, and commitment to quality care.
Maintain accurate records of referral activity, outreach efforts, contacts, and follow-up.

Experience working directly with hospitals, discharge planners, case managers, physicians, and senior care communities is highly valued.
3–5 years of healthcare experience preferred.
Direct experience in skilled nursing, rehabilitation, long-term care, or post-acute healthcare strongly preferred.
Bachelor's degree in Business Administration, Healthcare Administration, Marketing, or a related field preferred; equivalent healthcare experience will be considered.
Knowledge of the nursing facility admissions process.
Understanding of Medicare, Medicaid, managed care, insurance verification, VOB applications, and third-party reimbursement.
Strong knowledge of healthcare referral and discharge processes.
Excellent verbal and written communication skills.
Strong presentation, networking, follow-up, and organizational skills.
Professional, confident, and personable communication style.
Ability to establish trust and maintain long-term relationships with referral sources.
Ability to work independently while collaborating effectively with an interdisciplinary team.
Knowledge of applicable state and federal regulations related to nursing facility admissions is preferred.
