We are seeking a compassionate and highly skilled Home Care Nurse to join our dedicated team. The ideal candidate will possess extensive experience in various clinical settings, including hospital medicine, emergency medicine, and post-acute care. This role involves delivering comprehensive patient care in a home environment, utilizing advanced nursing skills, and ensuring adherence to healthcare standards. The Home Care Nurse will play a vital role in promoting patient well-being through thorough assessments, medication management, and personalized care planning.
<Responsibilities>
- Conduct detailed patient assessments to determine individual needs and develop tailored care plans.
- Administer medications, injections, and treatments following established protocols while ensuring patient safety.
- Monitor vital signs and physiological responses, documenting findings accurately within electronic health record (EHR) systems.
- Provide wound care, IV insertion, and infection control measures in compliance with healthcare standards.
- Educate patients and their families on health management, medication administration, and disease prevention strategies.
- Collaborate with physicians and interdisciplinary teams to coordinate ongoing care and adjust treatment plans as necessary.
- Maintain thorough medical documentation in accordance with HIPAA regulations and organizational policies.
<Skills>
- Proven experience working in Level II trauma centers, emergency rooms, or similar high-acuity environments such as ICU or PICU settings.
- Strong background in hospital medicine, medical-surgical nursing, or outpatient clinics with post-acute or sub-acute care experience.
- Knowledge of infectious disease management, trauma protocols, and emergency response procedures.
- Proficiency with EMR/EHR systems for accurate medical records documentation.
- Skilled in patient assessment techniques, physiology knowledge, anatomy understanding, and vital signs monitoring.
- Competent in medication administration including injections and venipuncture procedures.
