Job Summary
We are seeking a dedicated and energetic Midlevel Provider (Nurse Practitioner or Physician Assistant) to join our team at a leading Skilled Nursing Facility. - great part time opportunity for supplemental income. Flexible. Can make your own schedule. Will require regular availability via text and 2 to 3 days of rounding per week. notes must be completed within 1 week and can be done remotely .

Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine health status.
- Develop and implement individualized care plans in collaboration with physicians, nurses, and other healthcare professionals.
- Prescribe medications and treatments following state regulations
- Manage acute and chronic conditions common in skilled nursing populations, including geriatrics, dementia care, and complex comorbidities.
- Document all patient interactions accurately within Electronic Health Record (EHR) while ensuring compliance with HIPAA regulations.
- Participate in interdisciplinary team meetings to coordinate discharge planning, medication management, and ongoing patient education.
Qualifications
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) with appropriate state certification.
- Proven experience working in skilled nursing facilities or long-term care settings; experience in nursing homes preferred.
- Strong understanding of geriatrics, dementia care, hospice & palliative medicine, and post-acute care protocols.
- Ability to perform clinical evaluations involving physical examinations and diagnostic assessments like sonography or laboratory specimen collection.
- Excellent communication skills to collaborate effectively with healthcare professionals and educate patients’ families about treatment plans.

Pay: $80,000.00 - $115,000.00 per year
