Overview
We are seeking a detail-oriented, knowledgeable and motivated Licensed Practical Nurse (LPN) specializing in Home Care with a focus on compliance and chart review. This role involves ensuring accurate medical documentation, adherence to CMS (Centers for Medicare & Medicaid Services) Standards, and maintaining high quality patient care through meticulous review processes. The ideal candidate will possess strong clinical knowledge, familiarity with medical coding and experience in utilization review within Home Health settings.
Duties
- Review and analyze OASIS (Outcome and Assessment Information Set) data for accuracy, completeness, and compliance with regulatory standards.
- Conduct thorough audits of medical records to ensure documentation aligns with Medicare requirements and NCQA (National Committee for Quality Assurance) standards.
- Utilize EMR (Electronic Medical Record) systems such as HCHB (Home Care Home Base) to extract, review, and document clinical data efficiently.
- Collaborate with, educate and train clinical staff to verify proper documentation of patient assessments, and treatments.
- Apply knowledge of ICD-10/ICD coding classifications for accurate medical management documentation.
- Support compliance initiatives by identifying documentation deficiencies and recommending corrective actions.
- Participate in ongoing education related to healthcare regulations, HIPAA privacy standards, and clinical documentation improvement strategies.
- Responsibilities
Qualifications:
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Strong knowledge of Medicaid/Medicare regulations and guidelines
- Proven experience in Certified Home Health Agency (CHHA), and nursing environments.
- Familiarity with EMR/EHR systems, including HCHB.

Pay: From $92,000.00 per year
