Southern Tier AIDS Program, Inc. is looking for a Health Homes Care Manager to join our Binghamton team! This position provides confidential outreach, intake, and care management services to persons with qualifying chronic illnesses, as well as Health Homes member tracking tasks. The Health Homes Care Manager reports directly to the Health Homes Program Coordinator.
Responsibilities
The primary responsibilities and duties of a Health Homes Care Manager include:

Engaging clients in the performance of: outreach and location services, initial intake, assessment, reassessments, care plan development and implementation, follow-up interviews and contacts

Assisting with obtaining needed linkages and referrals, including: medical, legal, housing, social, financial, transportation, food security, and other requested services

Serving as client advocate, when necessary

Arranging case conferences with other health and human service providers as necessary

Coordinating all components of the care plan within the community

Providing crisis intervention activities for clients, including on-call responsibilities

Completing sufficient, accurate, and timely documentation of all interventions, activities, and communications during the above listed activities and all ongoing contacts with client. DOH requirements are three business days.

Enters and verifies the accuracy of member tracking data on outreach and enrolled Health Homes members to STCC, as well as lead Health Homes organizations. This may include: names, demographics, PHI (including Medicaid #s), dates of attempted contacts/enrollment/assessment, documentation of qualifying services, discharge, etc.

$19.24/hour working 35-hours per week, may vary depending upon experience
Education:

Associate’s degree (preferred) in Health, Education, Social Work, Liberal Arts, a related field, and/or two years of experience.
Experience:
-
Qualifying experience means verifiable full or part-time care management or customer service experience working with any of the following: persons living in poverty, persons with mental illness, homelessness, chemical dependence, HIV/AIDS, and/or other qualifying chronic conditions (diabetes, asthma, hypertension, etc).

Experience with documenting Medicaid billable services preferred.

Organizational skills, record-keeping, time management and communication skills are essential.

Computer literacy required.

Physical demands are those associated with general office duties including: sitting, standing and light lifting. This individual will travel in the community (i.e. visiting with service providers, client home visits, etc.).

This individual is responsible for meeting the transportation and insurability requirements of this job. A valid driver’s license, clean driving record and reliable transportation is a requirement of this position.
