Job Description:
Nina’s Health Care is seeking a dedicated and compassionate Physical Therapist (PT) to provide in-home therapy services to patients in need of rehabilitation and mobility improvement. As a Home Health Physical Therapist, you will work closely with patients in their home environment, helping them regain independence, manage pain, and improve overall physical function. Responsibilities:
- Provide hands-on physical therapy to patients in their homes based on personalized care plans.
- Assess patients' mobility, strength, and overall functional ability to develop effective treatment programs.
- Educate patients and caregivers on exercises, safety measures, and mobility techniques to promote long-term recovery.
- Document patient progress, treatment plans, and goals accurately and timely using electronic medical records (EMR).
- Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
- Utilize therapy techniques such as range of motion exercises, gait training, pain management strategies, and post-surgical rehabilitation.
- Monitor and adjust treatment plans as needed to meet patients’ evolving needs.
- Ensure compliance with state, federal, and agency guidelines for home health services.
Qualifications:
- Licensed Physical Therapist (PT) in VIRGINIA
- Experience in home health or outpatient physical therapy (preferred but not required).
- Strong communication and interpersonal skills.
- Ability to work independently while providing quality patient care.
- Valid driver’s license and reliable transportation for home visits.
