ACCA is seeking a compassionate and experienced Registered Nurse (RN) to join our Adult Day Health (ADH) team. This part-time position (24 hours per week) provides nursing oversight and care coordination for physically frail and/or cognitively impaired adults participating in a community-based medical model Adult Day Health program.
Our program provides individuals with an alternative to nursing home placement while giving families and caregivers valuable support.

Key Responsibilities
The RN will:
- Provide nursing oversight and supervise the delivery of nursing care to ADH participants.
- Complete initial and ongoing nursing assessments and evaluations.
- Develop, implement, and monitor individualized comprehensive care plans.
- Monitor changes in participants' health and communicate with physicians, families, care coordinators, and other appropriate providers.
- Review and maintain accurate clinical documentation and participant records in accordance with program and funding requirements.
- Coordinate care with physical, occupational, and speech therapy providers and other community service providers.
- Participate in discharge planning and coordination of services.
- Monitor vital signs, symptoms, medication compliance, weights, and other health indicators.
- Assist staff during medical emergencies.
- Supervise and support clinical staff and contribute to performance evaluation.
- Maintain positive relationships with participants, families, subcontractors, and community partners.
- Participate in required staff meetings, training, and professional development.
Qualifications
Required:
- Current Georgia Registered Nurse (RN) license
- At least one year of clinical experience in healthcare or social services
- Current First Aid/CPR certification
- Successful completion of required background check, drug screening, and physical
Preferred:
- Experience working with older adults
- Experience working with individuals with dementia or cognitive impairment
- Experience with chronic health conditions
- Experience in adult day health, home health, long-term care, or community-based healthcare
