Duties
- Conduct thorough patient assessments to identify psychosocial needs, barriers to care, and support systems.
- Develop individualized care plans that incorporate community resources, counseling strategies, and discharge planning.
- Provide clinical counseling including mental health support, behavioral therapy, addiction counseling, and end-of-life care discussions.
- Coordinate with healthcare teams to ensure seamless integration of social work interventions with medical treatment plans.
- Assist patients and families with navigating healthcare systems such as Medicare, Medicaid, and private insurance for coverage and benefits.
- Document all patient interactions accurately within electronic medical records (EMR) systems following HIPAA guidelines.
- Offer crisis intervention and behavioral health support for patients experiencing acute emotional or psychological distress.
Requirements:
- Master's Degree in Social Work
- Current Michigan License
- Home health experience preferred
- Reliable transportation and valid driver's license.
Pay: $80.00 - $100.00 per hour
