Admissions/Discharge Coordinator

We are seeking a compassionate and caring discharge planner to join us in our mission to ensure that our residents are living their very best lives. Key Job Responsibilities:
* Complete psychosocial history, mood, and behavioral assessments
* Meet with guests and families to assess discharge needs after rehab stay
* Identify potential barriers to discharge
* Meet with interdisciplinary team to determine recommendations to meet guest’s needs after discharge
* Assist guest in navigating and understanding their insurance benefits, approval process, appeals, etc.
* Provide tours of Ohio Eastern Star Campus to guests/families
* Communicate regularly in-person and by phone with guest and families to provide support and answer questions throughout their stay
* Communicate discharge plans to appropriate staff and coordinate transitions to other areas of our campus
*Facilitate the discharge plan by making referrals for home health/assisted living/SNF, order necessary DME, arrange follow-up appointments, coordinate transportation, and provide any additional resource information that could benefit guest after discharge
Preferred familiarity or experience with:
- Local services and providers: home health, SNFs, DME, etc.
- Insurance: Medicare, Medicaid, commercial, vendors/plans, prior authorization process
- PASRR and Medicaid LOC process
-Excellent communication skills and a team-oriented attitude
-Previous case management experience preferred.
