The Social Care Screener is a critical frontline member of the CABS Social Care Network (SCN), responsible for conducting Health-Related Social Needs (HRSN) screenings for Medicaid members. This role ensures accurate data collection, member engagement, and alignment with the SCN Operations Manual, contributing to the identification of unmet social needs and facilitating pathways for improved health equity.
General Responsibilities
- Conduct HRSN screenings using the Accountable Health Communities (AHC) Screening Tool or an SCN-approved interoperable platform, in compliance with SCN IT Platform requirements.
- Obtain informed consent from members for data collection and sharing, adhering to HIPAA and SHIN-NY guidance.
- Document screening outcomes, member demographics, and identified needs accurately and in real-time.
- Verify members’ Medicaid enrolment and eligibility for Enhanced HRSN Services during screenings.
- Maintain accurate records, ensuring compliance with all regulatory and organizational standards.
- Participate in training programs on HRSN tools, cultural competency, and SCN-specific protocols.
- Identify major life events or other qualifying criteria that warrant rescreening and update member profiles accordingly.
- Collaborate with Social Care Navigators and other SCN team members to ensure seamless handoffs and referrals.
- Work flexible hours, including evenings and weekends, as required to meet program demands.
Community Engagement
- Build rapport with Medicaid members to foster trust and engagement during the screening process.
- Actively participate in community outreach events to raise awareness of SCN services and identify potential members.
- Liaise with community-based organizations (CBOs) to address immediate social needs identified during screenings.
Continuous Evaluation and Improvement
- Prepare reports on screening metrics and outcomes, ensuring data accuracy and compliance with performance standards.
- Provide feedback on screening workflows to improve efficiency and member experience.
- Identify patterns in unmet needs and contribute to program evaluations aimed at closing service gaps.

Pay: $40,000.00 - $45,000.00 per year
